How to Apply for Assistance

  • The Foundation focuses its efforts on serving cancer patients residing in Northern New Mexico and seeking treatment in Santa Fe. The community served by the Foundation includes Santa Fe, Rio Arriba, Taos, and Los Alamos, Colfax, Mora, San Miguel, Union, and Harding counties. These counties were selected as Santa Fe is the primary oncology referral center for patients in Santa Fe and the identified counties in Northern New Mexico.
  • Patients may self refer for services or be referred by a treating health professional.
  • Patients complete a patient assistance application and provide supporting financial documentation.
  • The Patient Services Coordinator (Caroline Owen) completes an individual assessment to identify needs/eligibility/plan for the support services provided by the Foundation.
  • Eligibility is based on 300% of federal poverty guidelines. This standard is revised yearly by the federal government, and will be reviewed annually by the Foundation.


Assistance Provided:

Mileage Reimbursement: Qualifying patients who travel to Santa Fe for their treatment are eligible for mileage reimbursement.

Overnight Lodging: Qualifying patients who travel to Santa Fe for their treatment may be eligible for overnight lodging near the treatment center.

Grocery Cards: Qualifying patients currently undergoing treatment are eligible to receive grocery cards.

Support Groups: The Foundation offers several support groups to patients, caregivers, and loved ones at no cost. Click here for more information.
