How to Apply for Assistance

  • The Foundation focuses its efforts on serving cancer patients residing in Northern New Mexico and seeking treatment in Santa Fe. The community served by the Foundation includes Santa Fe, Rio Arriba, Taos, and Los Alamos, Colfax, Mora, San Miguel, Union, and Harding counties. These counties were selected as Santa Fe is the primary oncology referral center for patients in Santa Fe and the identified counties in Northern New Mexico.
  • Patients may self refer for services or be referred by a treating health professional.
  • Patients complete a patient assistance application and provide supporting financial documentation.
  • The Patient Services Coordinator (Caroline Owen) completes an individual assessment to identify needs/eligibility/plan for the support services provided by the Foundation.
  • Eligibility is based on 300% of federal poverty guidelines. This standard is revised yearly by the federal government, and will be reviewed annually by the Foundation.

Caroline-OwenMileage guidelines: Patients who travel to Santa Fe for their treatment and meet eligibility requirements receive mileage reimbursement calculated from their home to the treatment center.

Hotel guidelines:  Reservations are coordinated by Caroline Owen, the Patient Services Coordinator, and are provided to patients receiving treatment on consecutive days and for those living at least an hour away from the treatment center.
